To reach the custodian of records, please email records@usainfusions.com with your records request.  If the record is clinical in nature please indicate the client's name, date of birth, and phone number.  The release of any record will require a signed consent from the client, employee, or subcontractor.  Email records@usainfusions.com to obtain the correct consent form.
